Case Manager

The Developmental Disabilities Services Division Case Manager serves as a case manager for assigned clients at an assigned location. Within this framework, the case manager provides case management services, the components of which include screening, assessment, case planning, case review, advocacy, brokering, and providing follow-along services for persons who are at risk of institutionalization. Responsibilities may also include conducting necessary assessments, developing client-specific training programs, and staff/provider training.
